To ensure your application is assessed in a timely manner, return your application to the Department of Human Services (Medicare):

In person: At your local Medicare service centre or at any other Department of Human Services service centre.
If you use an older version of the CAPS application form, it may direct you to return your form to the Secretary of Department of Health. This will delay assessment of your application. Ensure you use the latest version of the CAPS application form and direct your form to the Department of Human Services (Medicare).
For assistance in completing the CAPS applications form, call Medicare on 132 011 or the CAPS Team on 1800 239 309.
Health Professionals should assist by using the latest version of the CAPS application form advising their clients to send completed CAPS application forms directly to the CAPS Team at the Department of Human Services.
